General Insurance Statement
Dr. Raskin does not accept health insurance of any kind and will be considered an out of network provider with all health insurance policies. We are a cash-based practice (pay at time of service) and Dr. Raskin has voluntarily opted out of all health insurance plans, including Medicare and Medicaid, and does not communicate in any way with health insurance carriers.
Medicare, Medicaid, And Tricare
Dr. Raskin has voluntarily opted out of Medicare, Medicaid, and TRICARE. All patients will sign an opt-out agreement with Dr. Raskin stating that the patient agrees to not submit any consultation fees incurred with Dr. Raskin to Medicare as a self-submission claim for reimbursement.
It is the patient’s responsibility to confirm with Medicare, Medicaid, and TRICARE that payment will be covered for lab costs ordered by Dr. Raskin.
Insurance
Although Kenneth Raskin, M.D. does not submit charges to your insurance carrier for reimbursement, you may submit these claims on your own. All insurance plans are different, and reimbursement depends solely on your policy. It is entirely at the discretion of the insurance carrier whether they will apply any invoiced charges to your deductible or reimburse you for those charges. Reimbursement is not guaranteed.
All invoices generated by our office will contain billing codes, description of service, diagnosis codes and provider identification information and can be used by the patient for self-claim submissions to their health insurance carrier (consultation fees cannot be submitted to Medicare, Medicaid, and TRICARE).
We do offer the option to create insurance-pay lab orders which offer our patients the option to use their health insurance card for payment when visiting the lab for testing. It is the patient’s responsibility to confirm with their health insurance carrier if any or all parts of the testing ordered by Dr. Raskin will be covered prior to completing the testing.
Because Dr. Raskin is an out of network provider, we strongly encourage you to contact your health insurance provider prior to completing any testing to determine your health insurance benefits. Once your blood has been drawn you will be responsible for any bill generated from the completion of those labs. We do offer a cash pay option for those who determine that they do not wish to use their insurance for lab testing.
Please be prepared for the possibility that the lab costs for your initial consultation lab order may not be covered by some health insurance carriers as Dr. Raskin will only be able to assign generalized diagnosis codes before your initial consultation. You can contact your health insurance carrier for clarification of their policy.
Cash Discounted Lab Orders Available
For patients who do not have health insurance or do not want to submit health insurance claims, Kenneth Raskin, M.D. offers very competitive cash discounted lab testing rates when using Quest Diagnostics for testing. Often this cash price is less expensive than utilizing your health care insurance.
